Managing Renal Dialysis at Home – A demonstration and discussion on peritoneal dialysis

Patients requiring renal dialysis have a choice of receiving their treatment in the Hospital or at Home. Peritoneal Dialysis is the most common form of home dialysis. Come to the Baxter stand to learn more about how peritoneal dialysis delivers care within the patient’s home, enabling patients to take control of their treatment.

A brief demonstration of the Long Term Conditions Disease Management Information Toolkit (DMIT): The Disease Management Information Toolkit (DMIT) has been developed to inform decision-making at both a strategic and local level. New Approach to Patient Self Care Education

Lancashire Condition Management Programme (Stand 55)

CMP Lancashire, established in 2004 was one of the seven initial pilot sites for Pathways to Work, CMP uses a number of mental and physical health interventions delivered by health care professionals working with jobcentreplus offices. The aim of the programme is to provide support in managing mild to moderate long term health conditions in order to facilitate a return to work. The programme is voluntary evidence based and has a clear outcome focus.
